  • Provide and coordinate targeted wellbeing support for students through an effective case management process.
  • Develop and maintain individual student support plans to improve wellbeing, engagement, and participation.
  • Build positive relationships to ensure students feel known, supported, and connected within the school community.
  • Guide students in exploring pathways and opportunities, supporting informed educational and career decisions.
  • Monitor student wellbeing and engagement data to identify students requiring additional support or intervention.
  • Liaise with families, school staff, and external agencies to coordinate appropriate support services for students.
  • Maintain accurate case notes, records, and documentation in line with departmental requirements.
  • Promote student resilience, inclusion, and positive wellbeing outcomes through proactive support strategies.
