Your Role:

As an Intensive Education Case Manager, you will:

  • Work collaboratively in a multi-disciplinary team to deliver intensive case management, provide transition supports as well as manage education, intervention and prevention initiatives relating to students who are at risk of disengagement, already disengaged or at risk of becoming involved with the youth justice system
  • Provide intensive case management that may require educational and behaviour intervention.
  • Establish and sustain effective partnerships with stakeholders, government and non-government organisations including families, education providers, community organisations and other key stakeholders.
  • Facilitate and lead the coordination of individual student plans and transition plans, using an integrated approach that consider a students' strengths and presenting concerns.
  • Implement best practice, culturally appropriate case management for all students to improve learning and engagement outcomes.
  • Maintain and record information management of activities across a number of platforms, in accordance with legislation, policy and procedures.

About You:

The ideal applicant will possess the following skills and abilities:

  • Ability to apply a strength-based, student-centred case management approach with young people from Prep to Year 12, and their families in a manner that respects their agency, culture, and perspectives.
  • Ability to develop and maintain effective working relationships with colleagues and stakeholders from a wide range of backgrounds, perspectives and systems to support the successful engagement and/or re-engagement of students.
  • Demonstrated ability to work in a complex operational environment where information and data must be managed, reported and shared appropriately and used to inform evidence-based decisions and actions.
  • Prepare and maintain high-level records and reports including case records, briefs, reports and correspondence for senior leaders.
  • Demonstrated ability and/or experience in the application of legislation, policy and procedure to achieve positive outcomes for young people.
  • Experience or the ability to rapidly acquire knowledge of relevant department procedures, operational processes, and educational systems to support students.

Highly desirable skills and knowledge:

Whilst there are no mandatory qualifications related to this role knowledge and experience of the following are desirable:

  • Understanding the impact of disability on individual functioning within an education context and evidence-based practices to support reasonable adjustments.
  • Experience working with First Nations communities, families and students and students from culturally diverse backgrounds.
  • Experience working with young people who demonstrate a range of support needs.
