Step Into Your Next Chapter:
As the Student Support Worker, you will play a key role in supporting upper years students with disability as they prepare for a successful transition to high school.
- Work closely with students to build confidence, independence, self-management and readiness for secondary school, while helping to create strong pathways of support for both students and families.
- Collaborate with teachers, school leaders, Student Services, families and external agencies to coordinate personalised supports and transition planning.
- Liaise with stakeholders, contribute to case management processes, and help ensure adjustments, strategies and supports are in place to meet student needs.

The Perfect Fit:
This role requires a proactive, organised and relationship-focused professional who can work as part of a team, communicate effectively with a range of stakeholders, and advocate for positive outcomes for students.
A strong understanding of disability support, inclusive practices and transition processes will be highly valued.
